These doctors can provide you with all of your primary care needs in your home the same way that your primary care doctor offers in his/her office. The difference is that the doctor comes to you. We have experience in working with individual doctors and Physician groups who make house calls. They differ from each other in the services they provide, the geographic regions they serve and the insurance they accept.

We will match you with as many doctors or groups in your area who accept your insurance We offer this referral service free of charge with no obligations. We do not accept payment from doctors either. The doctors do not work for us. This allows us to help you find a doctor who makes house calls who meets YOUR special needs.

How does a referral to a home visit doctor work? Once you make the on-line request or call, our concierge will ask you about your insurance, where you live and medical needs you have. Based on this information, our concierge will you a list of one or more recommended doctors. If you need help contacting a doctor you chose, our concierge can assist you.

PHYSICIAN SERVICES

Doctors who make house calls provide:

PRIMARY CARE SERVICES, EVALUATION, AND ASSESSMENT:

  • Medical exams and assessments
  • Evaluate and manage medications
  • Treat chronic diseases, including:
    • Congestive Heart Failure (CHF) and other Cardiac Conditions
    • Diabetes
    • High Blood Pressure
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Alzheimer’s and Dementia
    • Depression
    • Arthritis
    • Annual physicals
    • Wound Care
    • Other diseases treated by primary care doctors
  • Teach patients about their illness and medications
  • Vaccination

Some doctors will make urgent call visits within 48 hours of request.

COORDINATION OF CARE

  • Arrange for in-home nursing, therapy, medical social worker, and/or home health aide
  • Communicate and refer to specialists
  • Arrange for in-home visits by certain specialists, such as podiatrists and eye doctors
  • Arrange for in-home test by laboratory, x-ray, EKG, pulmonary function and others
  • Arrange for in-home nursing, therapy, medical social worker, and/or home health aide
  • Arrange for medical equipment
  • Hospice services
